What you will do:

* Conduct assessments and develop care plans tailored to individual patient needs within the hospital.

* Liaise effectively with healthcare professionals to ensure comprehensive support for each patient.

* Provide crisis intervention and emotional support to patients and their families.

* Facilitate discharge planning to ensure a smooth transition from hospital to community care.

* They will be working from Hospital 3 days a week, working 37 hours a week, Monday to Friday

* Maintain accurate case records and documentation in line with organisational policies.
