What you will do:

- Conduct comprehensive assessments of patients admitted for inpatient rehabilitation, ensuring detailed functional evaluations considering their home environments.

- Develop and implement tailored treatment plans focused on patient rehabilitation and successful discharge planning.

- Mentor junior staff, overseeing tasks with clear delegation and supervisory responsibilities.

- Attend and contribute to multidisciplinary team meetings, including whiteboard and remote meetings, ensuring a smooth patient flow on the ward.

- Ensure compliance with HCPC regulations, maintaining high standards of patient care and professional practice.

The role requires you to be registered with the HCPC, ensuring the delivery of quality care and adherence to professional standards.
