What you will do

* Conduct holistic assessments of social care needs and plan support across a dedicated caseload

* Collaborate with families, carers, and care workers to support people from diverse backgrounds with varied needs

* Assess and recommend major adaptations and specialised equipment to meet residents' needs

* Complete moving and handling risk assessments to ensure the safety and wellbeing of all parties involved
