What you will do:

* Manage a caseload of patients with neurological and community needs, assessing and treating communication and swallowing disorders.

* Collaborate with multidisciplinary teams to create comprehensive care plans for your clients.

* Conduct assessments and provide rehabilitation for stroke patients.

* Deliver therapy sessions in community settings, making your intervention accessible and impactful.

* Maintain accurate and timely records of patient progress and treatment outcomes.

* Educate patients, families, and carers on interventions to ensure continuity of care outside of sessions.

Requirements:

* Must be registered with the Health and Care Professions Council (HCPC).

* A minimum of 2 years experience in a relevant setting, with a focus on neurological conditions.

* A car driver is preferable to manage community visits efficiently.
