Key Responsibilities :

Conduct home visits for patient assessments, therapy provision, and equipment provision, working independently or alongside a dedicated team.

Collaborate with multidisciplinary teams (MDTs) to support urgent community responses and discharge processes.

Provide comprehensive occupational therapy services , ensuring each patient receives personalised and attentive care tailored to their needs.

Maintain accurate patient records , documenting progress and adjusting treatment plans as necessary to ensure optimal outcomes.

Requirements :

A full driving licence and access to a car is essential due to the community-based nature of the role.

HCPC registration is required to practice as an Occupational Therapist in the UK.

A minimum of two years' relevant experience , particularly in community-based settings.

Experience working with MDTs , supporting urgent care responses, and contributing to discharge planning processes.
