You will be working on an ongoing basis, providing specialized care to individuals living with dementia and enduring mental illnesses. What You Will Do:

* Assess, develop, implement, and evaluate care plans using evidence-based standards.

* Operate autonomously and know when to escalate care concerns.

* Provide guidance and direction to other staff in care delivery.

* Support patients in daily living activities as outlined in care plans.

* Act as a health promoter and educator for patients and their networks.
