What you will do:

* Provide care to patients referred by Sutton GPs, conducting home visits, care home assessments, and community engagements as needed.

* Assess and treat adult patients with neurological conditions, acquired communication, and swallow difficulties in the community.

* Apply your experience in managing dysphagia, ensuring that you are dysphagia trained.

* Engage in rehabilitation and long-term condition management, focusing on patients with conditions such as stroke, head injury, Parkinson's disease, multiple sclerosis, and other progressive conditions.

* Collaborate effectively within a multidisciplinary team and carry out community visits throughout Sutton in Southwest London.

* Ensure patients receive the best care by utilizing your EMIS training and NHS SMART card experience.

Requirements:

* Must be registered with HCPC.

* A minimum of two years' experience working with adult patients, particularly those with neurological conditions, is essential.

* Being a car driver and having a vehicle for this role is highly preferable to cover the extensive community visits required.
