* Facilitate joint assessments with partner organisations to foster a multi-disciplinary approach to care.

* Offer detailed information and guidance to clients and caregivers regarding available local services and supports.

* Maintain up-to-date and accurate records within the department's IT systems as a means for effective service delivery.

* Lead case work, providing ongoing advice and supporting vulnerable clients and their carers.
