What you will do:

* Develop and implement treatment plans for patients admitted to Elderly Medicine wards, using your expertise to facilitate recovery and rehabilitation.

* Collaborate with a multidisciplinary team to ensure seamless patient care and effective treatment strategies.

* Assist in planning patient discharges, ensuring that all necessary support and resources are in place for a smooth transition from hospital to home or other care environments.

* Conduct thorough assessments of patient progress and adapt treatment plans as necessary to achieve optimal outcomes.

* Educate and empower patients and their families, providing them with the knowledge and skills needed to continue rehabilitation outside the clinical setting.

Requirements of the Physiotherapist:

* Qualified physiotherapist, i.e., Degree or equivalent within Physiotherapy

* Recent UK experience within specialised physiotherapy

* Registered with the HCPC as a physiotherapist to work in the UK

* NHS experience would be advantageous but not essential
