With a competitive hourly rate of £26.00, this full-time role in Oxford allows you to work within a dynamic generic or specialist rehabilitation team managing a caseload for a three-month period. What you will do:

* Deliver safe and effective rehabilitative patient care, coordinating effectively with colleagues in both community and acute sectors to optimise resource use.

* Participate directly in the delivery of patient care, being a visible and accessible member of the team for patients, relatives, and colleagues alike.

* Work flexibly across localities and at weekends to meet service requirements, maintaining a strong presence within the team.

* Prioritise, assess, plan, and implement patient-specific programmes of care as part of an interdisciplinary team, applying your clinical reasoning skills to determine best practice.

* Engage with patients in setting their rehabilitation goals, ensuring informed consent is gained while working within the legal framework for those lacking capacity.

* Document and record assessments accurately within both organisational and professional guidelines.

* Evaluate patient progress, adapting treatment programmes as needed, and develop comprehensive discharge plans ensuring seamless care continuity.

* Undertake complex patient assessments as an autonomous practitioner, using clinical expertise to craft appropriate care plans.

* Support junior colleagues in risk assessments and maintain safety and quality standards across all service delivery.

* Contribute to service improvement initiatives focused on efficiency, quality of care, and the patient experience across the community team pathways.
