What you will do:

- Utilise clinical expertise to screen and process hospital discharges for the Discharge to Assess pathway.

- Support with trusted assessments where required, ensuring each patient receives quality and individualised care.

Requirements:

* Must be HCPC Registered

* Degree or equivalent within Occupational Therapy
