What you will do:

* Provide high-quality occupational therapy for individuals within the community rehabilitation setting, focusing on early discharge and maintaining independence.

* Conduct assessments and create individualized treatment plans for community rehabilitation and equipment provision.

* Offer guidance and support to clients in reviewing care packages, ensuring they meet the highest standards.

* Work collaboratively with a multidisciplinary team to ensure comprehensive and cohesive patient care.

* Engage in documentation and reporting on patient progress to facilitate successful rehabilitation outcomes.

Requirements:

* Registered with the HCPC.

* A minimum of two years' experience in community rehabilitation and early discharge.

* Skills in equipment provision and the ability to review care packages effectively.

* Car driving is preferable.
