What you will do:

- Conduct assessments in line with the Care Act to develop and review care plans.

- Implement the principles of the Mental Capacity Act, supporting people to make informed decisions about their care.

- Lead on S42 safeguarding enquiries, ensuring all actions are completed effectively and ethically.

- Work collaboratively with partner agencies to deliver a coordinated approach to care.

- Participate in the duty rota, providing expert advice and support as needed.

- Engage in risk management and employ a strengths-based approach to your work.

- Work closely with service users and families, employing person-centred planning to ensure their needs are met.

- Maintain accurate and concise records and ensure high standards of documentation.
