What you will do:

- Conduct initial patient assessments to identify individual needs and plan appropriate care pathways.

- Implement functional assessments to support effective discharge planning.

- Manage packages of care to ensure smooth transitions and continuity of therapy.

- Assess and prescribe equipment tailored to patient requirements.

- Liaise with community services to facilitate seamless discharges.

- Collaborate with multidisciplinary teams to deliver holistic care.

- Maintain accurate records and contribute to patient care reports.

Requirements:

You must be registered with the HCPC and possess at least two years of relevant experience within Acute Care in the NHS. A driving licence is preferable to assist in community liaison tasks.
