Job Description
Job Advertisement: Trusted Assessor - Nursing Homes
Location: Chertsey
Salary: £50,000 per annum
Schedule: Monday to Friday, 8:00 AM to 4:00 PM
We are seeking an experienced and proactive Trusted Assessor to join our team, supporting our nursing homes in delivering seamless and high-quality care for our residents. This vital role bridges the gap between healthcare providers and our home, ensuring residents' needs are fully understood and met during the admission and transition process.
Key Benefits:
- Competitive salary of £50,000 per annum
- £350 per annum training budget
- Yearly NMC fee reimbursement
- £1000 yearly retention bonus
Your Responsibilities:
- Ensuring patient assessments are completed in a 2 to 4 hour time window and their onward destination arranged and agreed between all relevant parties;
- Negotiating between care home managers and service users to facilitate discharge effectively to the agreed destination within 24- 48 hours;
- Partnership working with health and social care colleagues to maintain effective patient flow through the health and social care system;
- Ongoing relationship development with hospital discharge teams and the respective care setting management teams to raise the profile of the Trusted Assessor Role;
- Development of the Trusted Assessor pathway and according processes;
- Ongoing development of the Trusted Assessor role, identifying ways that current culture, methodology and processes can be improved;
- Ongoing communications with social care and commissioning colleagues to ensure the agreed block capacity is utilised in an appropriate and timely fashion, ensuring flow across the system.
- Development of performance reporting, regarding completed assessments, transfers to care homes, level of care home engagement etc
About You:
- Active / Valid NMC Pin
- Previous experience in a similar Trusted Assessor role or a senior position in healthcare/nursing homes.
- A professional background in nursing, social work, or healthcare management is highly desirable.
- Excellent communication, organizational, and interpersonal skills.
- Strong knowledge of care planning, health assessments, and regulatory frameworks.
- Compassionate and professional approach, with a commitment to resident dignity and well-being.
